W-9 - Non-employer Identification Certificate
Required to complete and retain with records for any non-employee your business issues a 1099-Misc.

Local Residency Certificate
Required Certificate of Residency for PA employers in compliance with Act 32. Employers are required to have all employees complete to determine proper local withholding. Retain form in employee records and inform employees to complete each time they move.
